We look forward to hosting your student this summer for our educational camp. In preparation for this experience, we are required by University of Rochester guidelines to provide the followings rules, procedures, as well as collect contact information for the student and their parents/guardians in the event of an emergency. All staff members (including student staff) at the Institute for Music Leadership have undergone the University’s training for working with minors, as well as emergency preparedness training.

CODE OF CONDUCT
This Code of Conduct is intended to promote community building and foster an environment conducive to the achievement of highest personal potential. If a student is unsure what this Code or any policy means, they have the obligation to ask. Compliance with this Code of Conduct is required of all students. Failure to comply may, at the discretion of the School, result in dismissal from the program. In the event of a significant violation, the Student’s parent or guardian will be contacted.
RESPECT OF OTHERS
Program participants shall be respectful of the Eastman School of Music community, which includes people with diverse backgrounds and beliefs. Conduct that is disrespectful or demeaning to others, including but not limited to verbal or physical harassment, will not be tolerated.
PROHIBITED ACTIVITIES
The following actions are prohibited during any part of the program:
- Participants may not leave university property or the program without permission of the Program Sponsor.
- Participants must follow all safety rules in accordance with university standards and/or as defined by the program administrator.
- Misuse of prescription drugs
- Possession, use, distribution, or being in the presence of alcohol or illegal drugs
- Use of tobacco products
- Possession or use of weapons (including–but not limited to–firearms, fireworks or other explosive materials, knives, or other items intended or commonly used to inflict harm) –any weapon found will be confiscated.
- Disorderly Conduct (including but not limited to):
- Fighting, including hitting or pushing another person, either as an aggressor or in response to actions of another
- Behavior that is otherwise reckless, disruptive, or dangerous to person or property
- Verbal or physical harassment of others including, but not limited to, bullying, cyberbullying and playing pranks or practical jokes, including via electronics
- Misuse, damage to, or theft of School property. Students will be liable for damage to Eastman School of Music property
- Tampering with or misusing fire or emergency equipment
- Use of a camera, cell phone or other video or audio recording device in spaces where privacy is expected, such as restrooms, showers, locker rooms and bedrooms being used by others • Failure to comply with the reasonable request of an Eastman School of Music official
- Interaction of a sexual nature or sexually suggestive manner with any other person (applies to any student under the age of 18).
While your student is participating in their program here at Eastman, they are allowed to have a cell phone with them to use to keep in contact with you. If they do not have a phone, or their phone isn’t working during their program, the named parent(s)/guardian(s) in this form may call either the Program Supervisor or the IML’s office phone number if they need to get in contact with their student (see next page). Because of federal privacy laws, only the named guardians should contact the Program Sponsor or IML staff for updates on the student, as we are unable to share any information about the student beyond those individuals. Only the primary parent/guardian can name other individuals with these permissions.
As part of the University of Rochester, the Eastman School of Music also has Public Safety officers on the premises. You can contact UR’s Public Safety dispatch/inquiry line by calling or texting (585) 275-3333. It is recommended that the student also add UR’s Public Safety number to their contacts if they have a cell phone. All activities during this experience will take place at the Eastman School of Music, primarily in the Miller Center. (Public Safety will ask for a location if you are calling for dispatch.)
In the event of an emergency, the student will be accompanied at all times at the IML office or another location on/near the Eastman School of Music campus (if the IML office is not accessible at the time). The student’s supervisor (or alternate, if unavailable) will notify the primary parent/guardian immediately in the event of an emergency, including medical emergencies, behavioral incidents, fire, natural disaster, and safety concern. Staff are to only to provide medical assistance in the event of a life-threatening condition or injury, or with consent from the parent/guardian.
